Integrated Care Fund Ideas and Innovation scheme

In July 2014 the Scottish Government launched the Integrated Care Fund. This fund relates to adult care and support services and is intended to support the delivery of improved outcomes from health and social care integration and help drive the shift of care towards prevention and tackling inequalities.

The ICF will only be available for 1 year (2015/2016). The amount of funding allocated to North Ayrshire through this fund is £2.89 million. The fund will support partnerships to focus on prevention, early intervention and care and support for people with complex and multiple conditions, particularly in those areas where multi-morbidity is common in adults under 65, as well as in older people.

We will particularly welcome ideas that will help to address health inequalities across North Ayrshire.
